Security Update - Fix DUO-PSA-2015-001

This release addresses DUO-PSA-2015-001. It also addresses issues #1 and #3

v2.0.1: resolve issue #2.

Fixes an issue where if a user opens a second tab, goes to a new site, gets sent back to the IdP for a session we will proceed with the new site rather than just sending a SAML failure.

This version requires MCB >= 1.1.2. The reason it's a major bump is because of bug-fixes in the MCB which caused us to change some code that was dealing with the MCB bugs which made this module incompatible with 1.0.0 MCB release.
